"THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY."

This is to notify you of federal laws governing the disclosure and use of client's personal health information. For continuity of care with this agency, the Department of Social Services, Home Health Agencies and the client's Physician and family, it is necessary to work together for quality care of you as our client. It is our professional obligation to preserve the privacy and confidentiality client information and its use.
The following information is necessary in identifying, establishing and continuing quality care of Handi-Care, Inc. clients:

  • Name and address
  • Social security, Medicare and/or Medicaid numbers
  • Date of birth
  • Telephone number
  • Physician's name
  • Diagnosis / health problems
  • Emergency contacts and telephone numbers
  • Information used for treatment, payment (deductibles, Medicaid or Medicare billing if applicable) or operational activities of the agency.

To help you in your daily care activities, necessary information is needed to be given to the Home Health Aide to allow this person to assist you in your needs. This information is in the form of a Plan of Care and ADL (activities of daily living) sheet to document tasks completed by the aide. The logbook containing this information is kept in the home, in a safe place and out of public view. At times it may be necessary to inform your social worker, home health nurse or physician regarding your care or changes in your condition. This is necessary so that your needs are met and for continued quality care.

We reserve the right to change this notice at any time and to make the revised or changed notice effective for personal health information that we already have as well as any information we receive in the future about you. Notice of change would be made available to you in writing.
